PREDIKSI KEKUATAN GENGGAMAN TANGAN BERDASARKAN PROFIL METABOLIK DARAH PADA POPULASI GERIATRI PANTI WERDHA HANA

Abstract

Latar Belakang: Kekuatan otot merupakan komponen fundamental fungsi fisik lansia, penting untuk mobilitas, keseimbangan, dan kemandirian dalam aktivitas sehari-hari. Tujuan: Penelitian ini bertujuan untuk mengurangi kesenjangan pengetahuan mengenai peran parameter glukosa darah puasa (GDP), HbA1c, kadar kolesterol total, kadar hemoglobin, dan kadar asam urat sebagai prediktor kekuatan otot pada lansia di Panti Werdha Hana. Metode: Studi observasional analitik dengan desain potong lintang ini melibatkan 55 responden berusia ≥60 tahun. Data diperoleh melalui pengambilan sampel darah vena setelah puasa minimal 8 jam dan pengukuran kekuatan otot menggunakan digital Hand Dynamometer Camry. Interpretasi kelemahan otot mengacu pada ambang batas normal >28 kg untuk laki-laki dan >18 kg untuk perempuan. Analisis data menggunakan kurva ROC dan uji Mann-Whitney, dengan tingkat kemaknaan statistik p<0,05. Hasil: Hasil menunjukkan bahwa mayoritas responden (81,8%) memiliki kekuatan genggaman tangan yang rendah. Analisis ROC memperlihatkan bahwa asam urat (AUC 0,770; p=0,008) dan HbA1c (AUC 0,720; p=0,031) memiliki nilai diagnostik yang signifikan dalam memprediksi kelemahan kekuatan genggaman tangan. GDP, kolesterol total, dan hemoglobin tidak menunjukkan nilai prediktif yang signifikan (p>0,05). Uji Mann-Whitney menunjukkan bahwa kelompok dengan kekuatan genggaman otot yang lebih baik memiliki kadar HbA1c (mean rank 40,15 vs 25,3; p=0,008) dan asam urat (mean rank 37,9 vs 25,8; p=0,030) yang secara signifikan lebih tinggi dibandingkan kelompok dengan otot lemah. Kesimpulan: Temuan kontraintuitif ini menyarankan adanya mekanisme kompleks atau efek perlindungan tertentu yang perlu diteliti lebih lanjut dalam konteks kekuatan otot pada lansia.
