Hubungan kadar D-dimer dengan mortalitas dan derajat keparahan pasien Covid-19 di RS Husada periode Januari - Desember 2020

Abstract

Infeksi Covid-19 menyebabkan kerusakan endotel dan hiperinflamasi serta reaksi berlebihan dari imun yang kemudian memicu kejadian badai sitokin. Hal ini menyebabkan peningkatan aktivasi koagulasi dan pembentukkan bekuan darah pada vaskular sehingga terbentuk D-dimer. Studi ini merupakan penelitian analitik dengan desain cross-sectional. Sampel yang digunakan berupa data rekam medis dari 348 pasien yang dirawat dan melakukan pengecekan D-dimer di RS Husada Jakarta selama periode Januari hingga Desember 2020. Hasil studi didapatkan tingkat keparahan penyakit terbanyak yaitu kriteria sakit sedang (183 pasien; 52.6%) dan ditemukan sebanyak 97 (46.9%) pasien mengalami peningkatan kadar D-dimer. Mortalitas pasien dengan peningkatan kadar D-dimernya yaitu 41 (19.8%) pasien (p-value 0,000). Pasien dengan tingkat keparahan berat/kritis memiliki kadar D-dimer yang meningkat yaitu sebanyak 110 (53.1%) pasien dibandingkan yang memiliki kadar D-dimer normal yaitu 55 (39.0%) pasien (p-value 0,001). Hasil studi menunjukkan hubungan antara kadar D-dimer yang meningkat dengan mortalitas dan tingkat keparahan pasien COVID-19.
