Abstract

Pasien geriatri memiliki tingkat kematian dan morbiditas tertinggi pada populasi operasi dewasa. Hal ini diakibatkan perubahan anatomis dan fisiologis tubuh sejalan dengan usia. Aneurisma sebagai salah satu penyakit serebrovaskular merupakan penyebab kelima kematian pada geriatri. Operasi clipping merupakan tindakan definitif terapeutik untuk aneurisma, dimana dibutuhkan relaksasi otak, memberikan tantangan bagi anestesi berkaitan dengan pencegahan hipotensi yang mengakibatkan iskemia otak. Tatalaksana anestesi meliputi sebelum operasi, selama operasi dan setelah operasi. Prinsip dasar neuroanestesi digunakan untuk mencegah terjadinya cedera otak sekunder dan ruptur aneurisma berulang. Penggunaan Total Intra Venous Anesthesia(TIVA) merupakan pilihan utama berkaitan dengan farmakologi obat beserta pengaruhnya pada neuron dan geriatrik. Evaluasi pra operasi yang dilakukan dengan hati-hati, manajemen anestesi yang tepat, operasi cepat dan atraumatik akan membantu mengurangi komplikasi perioperatif pada pasien geriatrik. Usia bukan dan tidak mengakibatkan kekurangan pada pasien bedah saraf geriatrik, yang faktor risikonya dikelola dengan tepat. Pasien geriatrik mendapatkan manfaat dari bedah saraf sama seperti pasien muda
